doc: fixes to please older versions of Texinfo

Currently, we have errors:

    ./doc/bison.texi:13005: node `History' lacks menu item for `Yacc' despite being its Up target

* doc/bison.texi (History): Add the local menu.
While at it, add a few index entries.
