Remove deprecated RGBASM features (#1215)

Browse Source 
- Escaped commas "\," inside strings
- `name: MACRO` syntax
- `__FILE__` and `__LINE__`
- `-H/--nop-after-halt` and `-l/--auto-ldh` on by default
